Student Information Sheet

The following is a helpful tool for the Office of Fellowship Advising, as well as for those writing recommendations in support of your scholarship application.  Think of this as a type of résumé, but one specifically designed to help your recommenders be very specific in writing letters for you.  On a separate sheet of paper, please provide information as it pertains to the following categories (Student Information Form ).

Course of Study

This includes your major(s), any minors, specific academic programs (Berry Scholars, Honors Program, CORE, Chaminade Scholars, etc.), as well as your current GPA.

Academic Accomplishments and Honors

Please list any scholarships, awards and recognitions for academic excellence while you have been a university student.

Research

Describe any research projects—past or present—which reflect your skill at carrying out significant scholarship in your chosen field of study.  This could be related to coursework, an independent study project, or work that you have completed with a faculty member or other professionals.

Relevant Work/Internship/Co-op/Volunteer Experience

Please list a brief description of any relevant experience.  For example, this could include working in a lab, volunteering for a political campaign, or interning or co-oping with a business.

International Experience

Experience and learning abroad can be important components of one’s education and often help to strengthen scholarship applications.  Please discuss any international travel experiences and what you gained from them.

Leadership

Leadership is almost always expected in scholarship competitions.  Please list your leadership activities and describe your specific role and contributions.

Service Activities

Service activities indicate good citizenship and an ability to empathize with others.  Describe how you have served your campus or other local communities.

Extracurricular Activities

Discuss any activities which demonstrate that you are a well-rounded person in addition to being a strong student.
